Description:

For over 25 years, Penn-Mar Organization has provided support services in our communities, which promote value, independence and self-determination, to families and individuals with disabilities or human service needs.

The Community Support Professional I will be responsible for a variety of duties related to the support of individual(s) within the Community Living Program who reside either in the community with family members or independently in their own home.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Obtain proper receipts and provide necessary documentation for all petty cash spent as well as assisting the individuals to obtain the necessary receipts for all money spent by them or on their behalf. Properly follow financial policy.
  • Assisting/teaching individuals general household responsibilities that includes but is not limited to cooking meals, completing laundry, cleaning, changing light bulbs, etc.
  • Provide safe transportation in personal vehicle to/from outings.
  • Maintain accurate mileage documentation for reimbursement and submit monthly to Supervisor.
  • Encourage an environment for individuals to express their personal choice and preferences while respecting their dignity of risk and remembering that safety is of the utmost importance.
  • Work in a safe manner, which includes wearing appropriate footwear and using proper lifting techniques, and be familiar with safety procedures. Encourage and teach safe procedures to consumers.
  • Establish and maintain daily progress notes documenting what was accomplished for each shift.
  • Attend weekly progress meetings with Community Living Manager.
  • Must establish a substitute base staff, which is trained to work with the consumer in the event hours need covered.
  • Responsible to assist with finding coverage for scheduled hours if unable to complete typical scheduled hours with individual.
  • Work with consumer on all formal goals and ensure accurate documentation is kept.
  • Ensure all data sheets are turned into the Community Living Manager on a monthly basis.
  • Assist in the preparation of the ISP for the individual.
  • Attend annual ISP meeting for the individual.
  • Maintain a cooperative relationship with the consumers, families and/or other caregivers within the Community Living Program.
  • Works in a safe manner, and promotes safety of the consumer.
  • Assist the Community Living Manager in any duties assigned.

This position is a full-time positionHigh school diploma or GED.
Valid Driver's License with 2 years of driving experience with less than 3 accidents or moving violations in the last 3 years.
Must pass basic Math and Reading Comprehension Exercise.
Able to lift 50 lbs.
Successful completion of physical, drug screen, Mantoux test for tuberculous.
Must pass background and reference checks.
